Webforms allow you to submit and sign paperwork without printing, attaching paperwork to an email, or physically returning the paperwork to the office.  It is automatically sent to the relevant party (Accessibility Resource Office/Testing Center/etc)

Webforms are recommended for forms that you are submitting to the Accessibility Resource Office.  A PDF is recommended if you are printing reference material or printing a form to physically return to the Accessibility Resource Office.  PDFs are currently being updated and will be made available as they are completed.
