Viewing Portable Document Format (PDF) Files

If you are unable to view the PDF form it may be due to the fact that you do not have the latest version of Adobe Reader. You may download the latest version free from Adobe Systems:

Using PDF Files

Adobe Reader will allow you to complete the forms on line and print them or you can simply print them and then fill them out manually.

If you choose to complete a form on line, simply place your cursor in the first field of the form to be completed and tab to succeeding fields (or use your mouse) until all the desired fields are completed.

Saving a completed PDF form

If you save a fillable PDF form, the information you've entered will not be saved. Adobe Reader does not allow users to save the entered information.

To save a blank PDF form

To save the PDF file:

• click on the Save a copy of the file (the disk icon) on the command bar on your browser.
• You can create a folder and save the copy as a PDF file into that folder using any name you choose. You can then reopen that file later and fill it out or print it
